The forms are usually hanging in a basket by the door in the main office, or you can simply go to the main office and ask for a form. They will give you a sheet you need to fill out get you parent’s signature, and a physical from doctor. You probably already have your physical done if you are in school or you play a sport!

Keep in mind: You can’t work more than three hours on school days. At 15 years old, when you get your working papers, you first get your blue card; but when you turn 16, you have to go to the office and fill out the sheet for the green card. This will allow you to work more hours. When you get your blue or green card, you can start applying to jobs. If you get an interview, bring you social security card and your blue or green so they know you are legally able to work in New York State.
